The End Stage Renal Disease National Coordinating Center (ESRD NCC) announces the launch of the #TogetherWeCan campaign. Each month the ESRD NCC website, http://esrdncc.org, will highlight true-life stories of patients and care partners that hope to inspire the kidney community to embrace life, no matter where they are in their journey.

The #TogetherWeCan campaign will feature stories about living with dialysis, receiving a kidney transplant, chronic kidney disease (CKD) and other aspects of the ESRD experience. The first in the series features Valen Keefer, a woman with polycystic kidney disease (PKD) and kidney transplant recipient, who is an inspirational speaker and advocate in the kidney community.

#TogetherWeCan encourages a sense of unity in the kidney community and promotes stories of hope in embracing life with CKD. If you or someone you know is interested in sharing an encouraging story for the #TogetherWeCan campaign, please contact the ESRD NCC at nccinfo@hsag.com.

The End Stage Renal Disease (ESRD) National Coordinating Center (NCC) provides support and coordination at the national level for the Medicare ESRD Network Program, under contract with the Centers for Medicare and Medicaid Services (CMS.)
