It was developed by the End Stage Renal Disease (ESRD) National Coordinating Center (NCC) with input from patient subject matter experts. Accessible from anywhere, this tool links to important informational resources such as:

• Transplant, home dialysis, hospitalizations, infection prevention, diet and nutrition, well-being, and new patient education.
• Additional features include access to the Patient Grant Library, an informative Understanding High-Kidney Donor Profile Index (KDPI) and Increased Risk Kidneys video, and more.
